Introduction: By learning how clinicians critically assess journal articles and translate this information into practice, the value of medical literature in clinical practice and its influence can be better understood. Methods: This study evaluated how physicians critically read information presented in journals and translate it into practice. Two hundred ninety-nine physicians participated in this online continuing medical education CME ; course. Participants rated their process of literature appraisal and its relevance to practice. Physicians were randomly assigned to read one of two articles Journal of the American Medical Association [JAMA] or International Journal of Clinical Practice [IJCP] ; , which presented primary trial data on therapy for the management of osteoporosis. They rated the impact of each article for relevance, educational value, applicability, overall opinion of the data, ability to identify key messages, and effect of article on their practice. Responses were compared to results obtained using a commercial literature evaluation service, the Astrolabe Message Mapping SystemTM AMMS ; . Results: Sixty-five percent reported spending 4 hr wk less reading articles. Physicians read the articles in a nonlinear manner. Top factors impacting perception, ranked from highest to lowest, were: quality of information, how well key concepts were supported by data, quality of journal, and format of presentation. Commercial sponsorship had the least impact on influencing credibility. Readers rated the methodology science of the JAMA article highest, which is consistent with the rating from the AMMS. JAMA readers identified the same key messages as the AMMS. More readers of the JAMA article indicated that the article would cause them to reconsider or alter their practice regarding osteoporosis treatment. Conclusions: Data presented clearly, with compelling scientific evidence and in a high-quality journal, have the greatest impact on clinical practice. Congruence between real-life ratings of the quality and impact of an article and the ratings from a commercial scoring process the AMMS ; was observed and luvox, because risperdal.
