Synopsis Eradication of Helicobacter pylori infection significantly reduces the risk of gastric cancer in individuals from regions associated with a high risk of gastric cancer, who do not have precancerous lesions, according to the results of a study published in the January 14th issue of the Journal of the American Medical Association JAMA ; . To determine whether treatment of H pylori infection reduces the incidence of gastric cancer, researchers conducted a prospective, randomised, placebo-controlled, population-based primary prevention study of 1630 healthy carriers of H pylori infection aged 35-65 years, who were from a high-risk region of China. A total of 1111 subjects agreed to have an endoscopic examination at baseline and a cohort of 988 participants were found to have no evidence of precancerous lesions gastric atrophy, intestinal metaplasia, or gastric dysplasia ; on study entry. Patients were randomly assigned to receive H pylori eradication treatment, which consisted of a 2-week course of omeprazole, 20 mg, a combination product of amoxicillin and clavulanate potassium, 750 mg, and metronidazole, 400 mg, all twice daily n 817 or placebo n 813 ; . Participants who had been randomized to receive triple therapy were invited to receive a carbon 13-urea breath test 13C-UBT ; 6 weeks after treatment by a standardised protocol. In subjects in whom eradication treatment failed were invited to receive quadruple therapy, which consisted of colloidal bismuth subcitrate, 240 mg, metronidazole, 600 mg, clarithromycin, 500 mg, and omeprazole, 20 mg, all twice daily for 1 week. A second 13C-UBT was performed 6 weeks after the second-line treatment period then regardless of treatment results, all participants underwent follow-up every 6 months which included a biannual 13C-UBTs for H pylori status, conducted by a local clinical team blinded to the treatment type of the participants. Five years after the first endoscopy, all randomised participants were invited to receive additional endoscopic examinations. The primary outcome measure for this study was a comparison of the incidence of gastric cancer during follow-up, between the H pylori eradication and placebo groups. A comparison of the incidence of gastric cancer in patients with or without precancerous lesions between the 2 groups was used as a secondary outcome measure. At the end of the follow-up period of 7.5 years, 18 new cases of gastric cancers had been identified and no overall reduction in the incidence of gastric caner was observed in participants who received H pylori eradication treatment n 7 ; compared with those who did not n 11 ; P 0.33 ; . However in the subgroup of patients with no precancerous lesions at baseline, a significant difference in the incidence of gastric cancer was found between subjects who had received H pylori eradication treatment compared with those who had placebo 0 vs. 6; P 0.02 ; . Smoking P 0.001 ; and older age P 0.001 ; were also identified as independent risk factors for the development of gastric cancer within this study group. The authors conclude that eradication of H pylori in patients with no precancerous lesions in high-risk areas is beneficial. However, whether the data can be applied to low-risk areas is unknown and warrants further investigation. In an accompanying editorial, Julie Parsonnet, MD, from Stanford University in California, and David Forman, PhD, from the University of Leeds in the U.K., review the guidelines and the current data and suggest that it is still unclear who should be screened or treated. They also highlight that while the authors found a 37% relative decrease in cancer incidence, the study was underpowered to detect such a difference, and they express other methodological concerns. They add that "Until more outcomes are available, individual clinicians will continue to do what we have for the past decade: use the very best informed judgment and hope for the best and azelaic.

In conjunction with winter weather and the higher probability of infection, not surprisingly, in a sampling of nearly 500 flavorings, Amoxicillin Clavulsnate Augmentin ; topped the list, followed by Azithromycin Zithromax ; and Amoxicillin Amoxil ; . These reports also indicate that greater numbers of pharmacies are receiving requests to re-flavor antibiotics that are already commercially flavored by manufacturers. This may be the result of several factors; however, initial studies indicate that it is most likely due to the sensitivity of a child's taste buds and also the result of children preferring the option to choose among a variety of flavors. Taste, especially among pediatric patients, is a key determinant in achieving medicinal compliance and taking medications properly. In addition, other components such as smell, texture and aftertaste also affect a child's willingness to swallow a medication. Taking medications exactly as prescribed by a pediatrician or healthcare practitioner are essential in antibiotic treatment programs. Failing to do so not only leads to treatment failures and poorer health outcomes, but also contributes to antibiotic resistance. In a 2004 article in the journal Infectious Diseases in Children, according to Dr. Edward Bell, PharmD, BCPS, "The risk of not completing a liquid antibiotic course likely increases with poor acceptance of taste by the patient." Taste assessments show that some liquid antibiotic products such as Amoxicillin have commonly scored well in taste tests that evaluate smell, texture, taste and aftertaste. Studies also noted that not all generic antibiotics of the same medication have similar tastes, and many are ranked neither as good or poor. As such, it can be deduced that taste preferences undoubtedly vary among patients, and even many so called 'better-tasting' medications still need to be re-flavored for many children. In the present series, a large proportion of the patients with NUTI 83.7% ; had urinary catheters and the most frequent risk factor was the use of urinary catheters, followed by diabetes mellitus, female gender and advanced age, a finding consistent with the literature 57 ; . Indeed, urinary catheterization increased the rate of NUTI by 3.83 times compared to the rate of other NIs. In addition, Candida spp, Pseudomonas spp, Enterobacter spp and methicillin-resistant coagulase negative staphylococci were isolated in specimens from patients with urinary cathe-ters but not in specimens from patients without a urinary catheter. Consistent with the results of numerous studies, this study revealed that Gramnegative bacteria and candida played an important role in development of NUTI, especially in patients with urinary catheters. The results of this study showed that the rate of resistance to widely used antibiotics was high for gram-negative bacteria. The most effective antibiotics against gram-negative bacteria were imipenem and meropenem. Cefuroxime, cotrimoxazole, ampicillin-sulbactam, amoxicillin clavulanate, gentamycin and tobramycin which are most frequently used for the treatment of community-based infections were the least sensitive antibiotics for the gram-negative microorganisms. The most important reason for resistance to antibiotics is the widespread use of antibiotics in hospitals.
